Website privacy

Effective August 11, 2026. This notice applies to the public Vested Health website and pilot application form.

Do not submit resident information, medical information, documents, or other protected health information through the public website. Pilot customers receive a separate, access-controlled application and contractual terms before any authorized data exchange.

Information we collect

The pilot form asks for a business contact's name, role, facility, work email, and an aggregate count of pending residents. Our hosting providers may also process ordinary security and request metadata, such as an IP address and browser information.

How we use it

We use this information to respond to pilot inquiries, evaluate fit, prevent abuse, secure the website, and maintain a record of business communications. We do not sell website lead information.

Service providers and retention

Website hosting and configured message-delivery providers process information on our behalf. We retain an inquiry only as long as reasonably needed for the business conversation, security, legal, and recordkeeping purposes, then delete or de-identify it.

Your choices

To request access, correction, or deletion of website contact information, email info@vested.health. We may need to verify the request and retain limited records where legally required.

HIPAA

This public website form is not an authorized channel for protected health information. Handling of information inside the Vested application is governed by the applicable customer agreement, business associate agreement, access controls, and customer instructions.
